The POI-2000 POE power injector is used to inject power onto a single network data cable being run to a peripheral device such as a network camera.
Network peripherals with POE capability built-in will be able to accept the network RJ45 data plug directly into their network socket, and via their built-in POE capability will separate the power and the data feeds within the device.
POE can also be used with peripheral devices which do not have POE capability built-in. This is simply achieved by using a POE splitter at the device to separate the data and the power onto separate cables for connection to their respective sockets.

You illustrate the 'base station module' end of the injector; the part the injects the voltage into the Cat5 cable. Is there a module at the camera end or does the CAT5 socket on the camera split the 12v from the camera signal?

If the IP camera is POE-enabled then you simply connect the data cable (with POE added), and the camera's internal electronics take the power from it.
If the network camera is not POE capable, then you can use a POE Splitter device to separate the power from the network cable and present the camera with the usual separate power and data leads. You need to select a splitter device which delivers the appropriate voltage for the camera.

Do you mean its IP address? In which case it does not have an address, it is transparent and merely adds power to the network cable.
Or do you mean its Ingress Protection rating? I cannot find a stated IP rating for this device, but would guess it to be about IP51.
